   Medical billing and coding translate a patient encounter into the languages healthcare facilities use for claims submission and reimbursement. Billing and coding are separate processes, but both are necessary for providers to receive payment for healthcare services. Medical coding involves extracting billable information from the medical records  and clinical documentation, while medical billing uses those codes to create insurance claims and bills for patients. Creating claims is where medical billing and coding intersect to form the backbone of the healthcare revenue cycle. For coding they use a set of specific codes. 

                       International Classification of Disease or ICD codes is the foundation for identifying health trends and is the international standard for reporting diseases and health conditions. Providers and hospitals around the world use ICD codes to classify disease, illness and injuries while insurance companies use that information to process claims. For instance. code H27.0 denotes Aphakia.

                             

  Aphakia means that you're missing the lens in your eye. The lens sits behind your pupil and iris which is the colored part of your eye. The lens focuses the light that comes into your eye and sends a focused image to the back of your eye. Without a lens to provide focus things will look blurry. Aphakia can affect anyone but it's most common in people who have surgery to remove cataracts.

Signs and symptoms of aphakia may include having:

• Blurred vision.
• Problems seeing things that are close and far away.
• Problems seeing the brightness of colors. Colors may seem to be faded. This isn’t the same thing as color blindness.
• An iris that jiggles. This is called iridodonesis.
• Problems adjusting to differences in how far away or close something is.

Aphakia treatments include surgery, contact lenses or glasses.
